How to get started with a new care package
Step 1: Get in touch
Following an informal chart with a member of our team, a home visit will be arranged as appropriate.
Step 2: Home visit and assessment
A qualified member of the team will visit you in your home to explain how our service works and get to know you better. During this visit the visiting officer will form an assessment of your individual needs and wishes, welcoming your family and informal carers to contribute to your tailored care plan.
Step 3: Care Plan Agreed
A care plan will be created based on your individual needs and requests as discussed during your home visit. This care plan will be reviewed by yourself and any other contributors to your care, in order to ensure you feel content with the planned care package. Once a care plan has been finalised , a start date will be agreed.
Step 4: Care services will commence
Your personalised care will begin whereby care staff will visit you each day, carrying out tasks agreed within your care plan.
Step 5: Review of the care plan
Your care plan will be reviewed annually, or sooner should any concerns or wishes be raised by yourself, family members or your care support team, or if there is a change in your health condition.
